** DO NOT SIGN THE SCHOOL'S VACCINATION WAIVER! **

It’s another school year and for everyone that is not aware yet, the following information is important!

It is incredibly important that parents do not sign the school or day care provided waiver/exemption form.  The language of the form is admission by the parent(s) of negligence because they are well aware of the risks posed by their decision.   Any signed document becomes a legal document. 

Only upon their educated refusal are parents offered the opportunity to exempt from vaccinations.  It is unfortunate that this is only afforded to parents on the basis that they sign a waiver which admits to negligence. You do not have to be one of those parents.  

View your state statutory laws at the National Vaccine Information Center.  Depending on what your state laws are, you may want to also at the vaclib site: http://www.vaclib.org/exemption.htm   For those states that have philosophical exemption laws the below general waiver form is available to print, fill, sign and submit to your school(s). This form can also be used for childcare.  The school administration and nurse's office will be well educated with this form.
